A Google Form QR code is a URL code. It does not store the form itself. It stores the link Google gives you when you click Send. Anyone who scans it lands on that form, the same way they would if you emailed the link.
Copy the form link
- Open the form in Google Forms.
- Click Send.
- Choose the link icon and copy the address. Google’s own short link from that dialog is fine.
- Leave the form set to accept responses, and check that the people you expect can open it (anyone with the link, or only your organization).
Turn the link into a QR code
- Open the QR code generator and stay on the URL tab.
- Paste the full
https://address. Do not swap it for a shortener you do not control. - Generate the code and download the PNG.
- Scan the image with your phone before you print. The form should open in the browser. No Google Forms app is required.
Print it
- Keep a light margin around the square and do not stretch the image.
- Use a dark code on a light background.
- Add a short label such as “Scan to fill out the form.”
- If you later stop accepting responses, or restrict who can reply, the code still scans. People just cannot submit.
The image is a static code. If you replace the form with a new link, print a new code. Editing questions inside the same form does not require a new code.
